Reference call/background checks

How strict are they? Do background checks mean they will ask my past employers for feedback? I am scared bc for my first internship, I had a sort of falling out with the manager bc I was sick for 2 weeks and she complained that 2 weeks is one fourth of the whole internship and I wasn't committed enough. In our final email exchange she said please don't hesitate to reach out for a recommendation but I am beyond terrified if they call her I am screwed.

 

FYI: The internships in question were unpaid and small firms so it’s even scarier as I can only list my manager. What if I write the reference’s # incorrectly and they can’t reach her - meaning they have to ask me for alternate proof (like the offer letter). Is this easier?
