you would ask for a letter of rec from the teacher/professor that knows you best. if not, then ask any of the previous teachers/professor you had that would be comfortable enough to write one for you, do a little research and see if any of them have ever worked for the place you are going to send in the letter of rec to, as it will be to your advantage. it is common courtesy when asking for a letter, that you ask at least a couple weeks (3-4) ahead of time. and dont forget to write a thank you letter after you have received the recommendation, regardless of whether you have gotten an offer or not.

Several of the top b-schools discourage recs from professors. They are usually looking for a rec from your immediate supervisor plus another...

isn't it one work related, one univ related...to cover two important bases, maybe a third relating to an EC/volunteer work (if it's worthwhile)
